LIABILITY WAIVER, RELEASE, AND CONSENT FORM

*Every child must have a completed waiver before entering the play area.

This quick and secure form helps us keep all children safe while they explore, play, and enjoy EveryKidz Corner. Parents or legal guardians must fill out the waiver for each child before their visit.

Participant (Child) Information

Assumption of Risk and Release of Liability

I, the undersigned parent/legal guardian, acknowledge that participation in activities at Every Kidz Corner involves inherent risks (including physical activity, use of play structures and sensory equipment, and interaction with others). I voluntarily assume all risks of injury, illness, or property damage that may arise from my child’s presence or participation.

To the fullest extent permitted by law, I hereby waive, release, and discharge Every Kidz Corner, its owners, agents, employees, contractors, volunteers, and affiliates from any and all claims, liabilities, demands, causes of action, or damages (including those arising from ordinary negligence) related to my child’s attendance, presence, or participation on the premises or in any program, except for claims arising from gross negligence or willful misconduct as defined by applicable law.

Medical Authorization

If my child requires emergency medical treatment and I cannot be reached, I authorize Every Kidz Corner staff to obtain necessary medical care and to transport my child to a medical facility. I accept financial responsibility for such care and transport.

Parent/Guardian Supervision Acknowledgement

I understand that Every Kidz Corner is not providing childcare during general open play; parents/guardians are responsible for supervising their children unless otherwise stated for a specific event or private session. I agree to supervise my child and ensure compliance with posted rules and staff directions.

Acknowledgement of Rules & Conduct

I agree my child and I will follow center rules and staff instructions. I understand the center may refuse entry or ask individuals to leave for safety or behavioral reasons and that fees/rules for refunds or credits are set by management.

Confidentiality and Recordkeeping

I authorize Every Kidz Corner to retain the above information for records and to contact me regarding my child’s participation. Records will be stored in accordance with the center’s privacy practices.

Governing Law

This agreement is governed by the laws of the state in which Every Kidz Corner operates. If any part is found unenforceable, the remainder will remain in effect.

Acknowledgement and Signature

I have read, understand, and agree to this Waiver, Release, and Consent. I sign voluntarily and certify I am the parent or legal guardian of the child named above.
